GAP Exceptions - BloomCare Billing LLC does not complete or submit insurance-specific forms for GAP exception requests. It is the client’s responsibility to locate, complete, and submit any forms required by their insurance company. This service includes preparation of a written justification and applicable CPT codes to support an in-network exception request only. GAP exception requests must be submitted before the birth. Approval is not guaranteed, and final determinations are made solely by the insurance provider.
