Insurance Ready Superbills

For Home Birth & Out-of-Hospital Birth Families

The Documentation You Need to Request Insurance Reimbursement

If you paid your midwife directly, your insurance company may require an itemized superbill to review your request for reimbursement.

BloomCare Billing prepares insurance ready superbills specifically for home birth, midwifery, and newborn care. We organize, code, and format your documentation based on the information and records you provide, helping you prepare a professional document to submit directly to your insurance company.

What Is a Superbill?
A superbill is an itemized document used to request insurance reimbursement for healthcare services. Depending on the services provided and the documentation available, it may include:
• Diagnosis codes and applicable procedure codes
• Patient and provider information
• Dates of service
• Itemized charges
• Other billing details required for insurance review

You submit the completed superbill directly to your insurance company for consideration under your plan's out of network benefits.

What's Included?

Every superbill package includes:
• Applicable ICD 10 diagnosis and CPT procedure coding based on the records provided
• Itemized insurance ready formatting
• Separate mother and baby documentation when applicable to the package
• Your completed superbill delivered as a Word Doc or PDF by email
• Simple instructions for submitting your superbill to your insurance company

Provider information: You are responsible for providing the required patient and provider details. These may include your midwife's NPI, tax identification number, practice address, service dates, and billing information. You may need to obtain these details and supporting records from your midwife or their patient portal.

BloomCare cannot supply missing provider credentials. Missing or inaccurate information may affect your insurer's review. Your superbill is prepared using the documentation you provide. Insurance companies may have additional requirements, and reimbursement is not guaranteed.

How It Works

1. Choose Your Superbill Package
Continue to our order form to select the superbill package that fits your needs and any optional services you would like to purchase. Pricing is displayed on the order form.

2. Upload Your Information and Records
After your purchase, BloomCare will email you a secure link where you can upload the required insurance information, patient records, and midwife documentation needed to prepare your superbill. Please provide all requested information and supporting records so we can prepare your documentation accurately.

3. BloomCare Prepares Your Superbill
Once we receive your uploaded information and required records, we will review the documentation and prepare your itemized, insurance ready superbill. If essential information is missing or clarification is needed, we may contact you before completing your document.

4. Receive Your Completed Superbill
Your finished superbill will be emailed to you as a PDF, ready for you to submit to your insurance company.

5. Submit Your Superbill to Insurance
You can generally submit your superbill in one of two ways, depending on your insurance company's procedures.

Option 1: Submit online
Log in to your insurance company's member portal and look for an option to submit an out of network claim or request reimbursement. Follow your insurer's instructions and upload your superbill along with any additional documents required.

Option 2: Submit by mail
Mail your superbill and any required supporting documents to the claims mailing address listed on the back of your insurance card. Check your insurer's instructions to confirm whether a separate claim form or additional documentation is required.

Please note: BloomCare prepares your superbill, but you are responsible for submitting it to your insurance company and responding to any additional requests from your insurer.

Important Information
• A superbill is documentation used to request reimbursement. It does not guarantee coverage or payment.
• Your insurance plan determines eligibility, reimbursement, and applicable deductibles or coinsurance.
• You are responsible for providing complete and accurate patient, provider, and billing information.
• Verification of Benefits, claim preparation and submission by BloomCare, insurance follow up, and appeals are separate services and are not included in superbill preparation.
• Preparing a superbill does not mean that BloomCare has submitted an insurance claim on your behalf.
• All sales are final once document creation has begun, subject to applicable law.

Is Superbill Preparation Right for You?
This service may be right for you if:
• You paid your midwife directly for home birth or midwifery care.
• You want to submit your own reimbursement request.
• You need professional, itemized insurance documentation.
• You are comfortable submitting the completed superbill through your insurance portal or by mail.
• You do not need BloomCare to submit or manage your insurance claim.

Need More Help With Insurance?

If you would prefer BloomCare to contact your insurance company to verify reported benefits or prepare and submit your insurance claim, we offer those services separately.

You can start with our Verification of Benefits service for $49. If you proceed with claim preparation and submission within six months of receiving your VOB summary, your $49 payment is credited toward the $100 claim preparation and submission fee.

Superbill preparation and full service claim support are separate services. Choose the option that best fits your needs.
